Individual Options Waiver Services & Level One Waiver Services
DD Waiver Nursing
Depending on your pre-determined service plan, we will send nurses and/or aides to provide direct care. This waiver is designed to give support services to individuals with intellectual and developmental disabilities to unlock each individual’s full potential and grow as active members of society.
Homemaker Personal Care
Loving Care Transitional Home provides support, services, and supervision for the health and welfare of every individual we serve, empowering them to live independently with a well-maintained quality of life. We have non-clinical staff who is responsible for supplementing the clients and their families with their self-care and daily living tasks.
We offer assistance with:
- Getting in and out of the bed
- Bathing and showering
- Meal preparation and feeding
- Personal hygiene and grooming
- Transferring
- 24/7 care
- Companionship
- Light housekeeping

HPC Transportation
With the right HPC Transportation Waiver Service Delivery Documentation, we can take you comfortably to and from your medical appointments. Whether you need a mode of transport in going to a rehabilitation center or during a medical discharge, we arrange convenient and safe transportation services for you and your loved ones.
Community Inclusion
We believe that community participation is an essential element in supporting people with disabilities. Being socially involved and taking part in community activities is an integral part of our care. This way, the care recipient can feel emotionally and physically secure with developed connections, friendships, and a sense of belongingness.
Personal Care Aide (TDD Waiver Only)
We know how paramount are your self-care needs to achieve optimum wellness. Your health reflects your way of living and we understand how your condition limits you from maintaining it. Our caregivers will be there to help you with hygiene care, various daily living activities, and routine tasks.
Supported Living
We give you a sense of independence and support to control your life. For you to have an enhanced way of living, we can help you in managing your bills or finances, cooking, getting a job, shopping, teaching new skills, managing medication, and accessing sports, employment, and social activities.
Shared Living
We help you develop a supportive, strong, and lasting relationship with others. Our shared living program offers our clients an opportunity to become connected to and be embraced by the community, experience a higher level of independence, build new skills, and foster unique interests. This elevates the condition of individuals with disabilities by engaging them in social life.
Adult Family Living
We support adults to age in place while receiving assistance from a personal caregiver. Adult family living program provides an option to deal with the daily challenges of aging. The care provider can be a relative of the care recipient as long as they are legally liable relative or licensed to do so.
Adult Foster Care
We provide 24-hour living support and arrangement with adult’s daily supervision. This refers to the care provided to persons in a home-like environment. It is basically non-medical in nature and involves ongoing assistance with the activities of daily living.
